Posterior Perspective of the Left Anterior Basal Segmental Bronchus

Description

Arising from the left inferior lobar bronchus, the left anterior basal segmental bronchus (bronchus segmentalis anterobasalis sinister, B8) courses anteroinferiorly within the lower lobe but is traced here from a posterior perspective. From this back view, B8 lies medial to the lateral basal segmental bronchus (B9) and typically anterior to the posterior basal segmental bronchus (B10), with the more superior segments of the lower lobe situated cranially. Segmental branching patterns and their relationship to the oblique fissure can be appreciated as the bronchus advances distally toward the anterior basal parenchyma. Posterior orientation matters in thoracic anatomy because it matches the operative and bronchoscopic reality: segmental bronchi are approached along the bronchovascular sheath, and the surgeon or bronchoscopist often thinks in terms of what sits anterior versus posterior to the segmental origin. Correctly identifying B8 helps prevent wrong-segment resection during a left lower lobe segmentectomy and supports planning of a posterior or posterolateral VATS approach, where the lower lobe is mobilized and the oblique fissure is developed to isolate segmental structures. It also aids interpretation of bronchoscopy and CT when mucus plugging, aspiration-related consolidation, or post-obstructive atelectasis localizes to the anterior basal segment.
