Viewed from an inferior perspective within the left lung hilum, the left superior lobar (secondary) bronchus (bronchus lobaris superior sinister) occupies a superior and lateral position relative to the left inferior lobar bronchus and courses distally before dividing into segmental bronchi. From this underside angle, the branching pattern into the apical, posterior, and anterior segmental bronchi of the left upper lobe can be appreciated, with the lingular bronchus arising anteroinferiorly toward the superior and inferior lingular segments. Cartilaginous bronchial walls and the lumenal bifurcations form the key landmarks. Orientation is everything. For bronchoscopy teaching and thoracic surgery planning, an inferior presentation clarifies why the lingular takeoff is often encountered early when the scope is advanced into the left upper lobe, and why secretions and aspirated material can preferentially track into dependent subsegments in supine patients. Segmental anatomy also underpins oncologic resection: a left upper lobe segmentectomy demands confident identification of the bronchial branch to be divided while preserving adjacent segmental bronchi, and this view aligns with the operative perspective during hilar dissection and stapled bronchial division.
