Description
Seen from a superior (cephalad) orientation, the left main bronchus is followed into the left superior lobar bronchus, with emphasis on the anterior segmental bronchus of the upper lobe (B3, bronchus segmentalis anterior). The branch courses anterolaterally within the superior lobe parenchyma, positioned anterior to the apicoposterior segmental bronchus (B1+2) and superior to the lingular segmental bronchi (B4 and B5). Segmental anatomy is implied by the bronchial arborization, with proximal lobar divisions giving way to more distal, caliber-reduced segmental airways. For thoracic surgeons and interventional pulmonologists, isolating B3 matters because it defines the bronchopulmonary segment most often targeted for upper lobe segmentectomy when a small peripheral lesion is confined to the anterior segment, and it is also a common route for selective bronchoscopy and endobronchial navigation. A superior view clarifies how the left superior lobar bronchus divides early, so a minor rotation during bronchoscopy can redirect the scope from B3 into B1+2 or down toward the lingula. Orientation errors happen here.
