Description
Anteriorly oriented pulmonary anatomy is presented with emphasis on the left main bronchus continuing into the left lobar (superior) bronchus and its anterior segmental bronchus (bronchus segmentalis anterior) supplying the superior lobe. Superior and lateral to the left hilum, the segmental bronchus courses within the parenchyma toward the ventral portion of the upper lobe, while adjacent pulmonary vessels track alongside as segmental branches. Medial relations remain anchored at the root of the lung, where bronchus, pulmonary artery, and pulmonary veins organize around the hilum in their characteristic anterior to posterior sequence. Accurate identification of the anterior segmental bronchus matters in bronchoscopy and thoracic surgery, where segment-based localization guides targeted suctioning, lavage, and resection. Segmentectomy of the left upper lobe depends on understanding the bronchovascular pedicles and intersegmental planes, and misidentification can compromise margins or injure accompanying arterial branches. A frequent teaching point: endobronchial lesions or retained secretions can obstruct a segmental bronchus and produce segmental atelectasis that respects anatomical boundaries on imaging. Pulmonology and anatomy courses use this view to teach the hierarchy from main to lobar to segmental bronchi, and to correlate bronchopulmonary segments with CT and bronchoscopy reports.
