Description
Anteriorly oriented anatomy centers on the distal ductus choledochus (common bile duct) as it approaches the wall of the pars descendens of the duodenum, where the inferior sphincter fibers form a sleeve around the intramural segment. The distal bile duct lies to the right of the pancreatic duct in the pancreatic head region, converging toward the hepatopancreatic ampulla (ampulla of Vater) at the major duodenal papilla. Circular smooth muscle bundles encircle the duct near its termination, positioned inferior to the more proximal choledochal sphincter components and medial to the second part of the duodenal lumen. That distal cuff of smooth muscle matters because it is the gatekeeper for bile delivery and a frequent culprit in biliary obstruction when functionally hypertensive. Small changes in tone at the sphincter of Oddi can reproduce postcholecystectomy biliary-type pain and transient cholestatic liver enzyme elevations, and they complicate ERCP cannulation by resisting passage into the bile duct. A tight segment here also changes the way a stone behaves at the ampulla, which is why choledocholithiasis may present with intermittent jaundice or pancreatitis depending on whether the common channel is involved. Short segment, big consequences.
