Description
Situated at the distal end of the common bile duct, the inferior sphincter of the bile duct (sphincter choledochus) encircles the lumen just before it meets the duodenal wall. Surrounding smooth muscle fibers blend with the sphincter of the pancreatic duct (sphincter pancreaticus) and the hepatopancreatic sphincter (sphincter ampullae hepatopancreaticae, sphincter of Oddi) as the ducts converge at the hepatopancreatic ampulla (ampulla of Vater). The segment lies inferior to the liver and gallbladder and medial to the second part of the duodenum, where the major duodenal papilla marks the mucosal opening. Small structure. High consequence. Control of biliary outflow at this level underpins many day to day clinical problems. Functional obstruction or spasm of the sphincter of Oddi can cause biliary colic-like pain and transient cholestasis even without gallstones, while an impacted stone at the ampulla can obstruct both bile and pancreatic juice, a classic trigger for gallstone pancreatitis. Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy targets this muscular complex, so clear depiction of the sphincter choledochus in relation to the ampulla and duodenal wall matters when teaching procedural anatomy and explaining post-ERCP complications such as bleeding or perforation.
