Description

Oriented from an inferior aspect, the hepatobiliary tree is visualized at the porta hepatis, where the right and left hepatic ducts converge to form the common hepatic duct, with the cystic duct from the gallbladder joining to create the common bile duct. The gallbladder lies on the visceral (inferior) surface of the liver, its fundus projecting anteriorly while the neck turns medially toward the hepatoduodenal ligament. In keeping with the semi-transparent rendering, the duct lumina can be followed as green branching channels within adjacent parenchyma, and the distal common bile duct is traced toward its junction with the main pancreatic duct near the second part of the duodenum. Spatial relationships matter here. Inferior views are where the surgical anatomy becomes unforgiving, because the porta hepatis is the gateway to the portal triad and small variations in ductal confluence can change a cholecystectomy from routine to reconstructive. A short cystic duct, a low union with the common hepatic duct, or a duct of Luschka in the gallbladder fossa are all patterns that predispose to postoperative bile leak or iatrogenic common bile duct injury. Following the common bile duct to its meeting point with the pancreatic duct also anchors the clinical logic of obstructive jaundice and gallstone pancreatitis, where a stone impacted near the ampulla can block bile flow and trigger pancreatic duct hypertension. Anatomy drives the complications.
