Presented from a posterior aspect, the visceral surface of the liver is oriented to highlight the extrahepatic biliary tree as it emerges from the hepatic parenchyma and descends toward the duodenum. Right and left hepatic ducts converge to form the common hepatic duct, joined inferiorly by the cystic duct from the gallbladder to create the common bile duct. The gallbladder lies inferior to the right lobe, while the porta hepatis region sits superior to the first portion of the common bile duct, where adjacent vascular channels correspond to the portal vein and hepatic artery proper in their expected relationships. Spatial relationships are clear. Posterior positioning matters because it clarifies the retroduodenal course of the common bile duct and its proximity to the hepatoduodenal ligament, a relationship that drives both imaging interpretation and operative strategy. Stones migrating from the gallbladder into the common bile duct (choledocholithiasis) commonly lodge near the distal duct, producing obstructive jaundice and pancreatitis when the ampullary region is involved. Surgeons and endoscopists think in these planes during laparoscopic cholecystectomy and ERCP, where misidentification of the cystic duct versus common hepatic duct is a classic mechanism of iatrogenic bile duct injury.
