Description

Anteriorly, the semi-transparent liver is rendered to expose the intrahepatic biliary tree as green branching channels that converge from peripheral segmental ducts into right and left hepatic ducts near the porta hepatis. Inferior to the right lobe, the gallbladder lies on the visceral surface, its fundus projecting anteroinferiorly while the neck tapers medially to become the cystic duct. The cystic duct joins the common hepatic duct to form the common bile duct, which descends inferiorly toward the second part of the duodenum, with the expected relationship to the pancreatic head implied distally. Orientation matters here because an anterior overview clarifies the extrahepatic confluence and the bottlenecks where obstruction and iatrogenic injury cluster. This is the setup for choledocholithiasis and acute cholangitis, where a stone impacted in the distal common bile duct produces upstream ductal dilatation, jaundice, and pain; it also frames the classic cholecystectomy hazard zone around the cystic duct and common hepatic duct, where misidentification can lead to bile duct transection or late stricture.
