Description
Oriented from an inferior perspective, the left inferior lobar bronchus is seen giving off its first segmental branch, the superior segmental bronchus (B6) to the superior segment of the left lower lobe. Proximally, the bronchial lumen and cartilaginous wall frame the airway, while the branch courses superiorly and posteriorly relative to the remaining basal segmental bronchi. Posterior trachealis muscle fibers and the posterior wall of the trachea may be included as a comparative landmark for airway orientation and depth. B6 matters because it marks a consistent anatomic gateway into the lower lobe: the superior segment sits posterior and superior to the basal segments, and disease here can behave differently from dependent basal pathology. Aspirated material more often tracks into the right lower lobe, but the left superior segment is a familiar site for postobstructive atelectasis or localized pneumonia when a mucus plug or foreign body lodges at an early segmental division. Bronchoscopic navigation also benefits from this angle, since the inferior view clarifies how a superiorly directed takeoff can be missed if the scope is kept too anterior. Small branch, big consequence.
