Description
Centered in the lower anterior neck, the thyroid isthmus spans transversely between the right and left lobus glandulae thyroideae, forming a thin parenchymal bridge that crosses the anterior tracheal rings. The lobes sit lateral to the trachea and inferior to the laryngeal framework, while the isthmus lies medial and slightly inferior to their maximal bulk. In this anterior presentation, the isthmus is the defining midline element, with its superior border approaching the level of the cricoid cartilage and its inferior border tracking toward the suprasternal region. Surgeons and radiologists care about the isthmus because it marks the midline thyroid tissue most directly related to the airway, and its variable height and thickness affect both palpation and operative planning. Prominent isthmic tissue and a pyramidal lobe can explain a midline neck mass that rises with swallowing, and the isthmus is commonly divided during thyroidectomy to mobilize each lateral lobe while protecting the anterior tracheal wall. Needle biopsy and ultrasound reporting often separate isthmus nodules from lobar nodules, since isthmus lesions sit close to the trachea and can present with early compressive symptoms or anterior strap muscle invasion.
