Description
Bridging the right and left lobes of the glandula thyroidea, the thyroid isthmus spans the anterior midline of the lower neck, typically overlying the second to fourth tracheal rings. Superior to it lies the thyroid cartilage and cricoid region, while inferiorly the tissue approaches the suprasternal notch and upper mediastinal inlet. Laterally, the isthmus blends into the medial surfaces of each lobe, with its anterior surface invested by pretracheal fascia and overlaid by the infrahyoid (strap) muscles. A small pyramidal lobe may project superiorly from the isthmus toward the hyoid. Surgical anatomy of the isthmus matters because it sits directly on the trachea, where small midline veins and variable arterial twigs can complicate exposure and hemostasis during thyroidectomy. Midline division (isthmusectomy) is a common step to mobilize the lobes and identify the anterior tracheal wall, and the relationship to the prelaryngeal (Delphian) lymph node can influence oncologic staging in papillary thyroid carcinoma. Goiter and thyroiditis often thicken this bridge of tissue, narrowing the operative corridor. Expect variation.
