Seen from the lateral aspect, the uterine body and cervix lie superior to the vagina, with the vaginal canal angling posterosuperiorly toward the cervix and forming the anterior and posterior fornices around it. Anterior to the cervix and upper vagina sits the vesicouterine region and bladder interface, while the rectum and rectouterine pouch lie posteriorly, separated from the posterior vaginal wall by the rectovaginal septum. Inferiorly, the vagina traverses the pelvic floor at the level of the levator ani, with the perineal body positioned posterior to the distal vagina and anterior to the anal canal. This side view matters when you need to explain how support failure translates into symptoms. Descent of the anterior vaginal wall with loss of pubocervical fascia support produces cystocele and stress urinary incontinence, while posterior vaginal wall laxity along the rectovaginal septum contributes to rectocele and obstructed defecation.
