Description

Anterior female pelvic anatomy is represented with the uterus positioned superior to the vagina, the cervix projecting inferiorly into the vaginal canal to create the anterior, posterior, and lateral vaginal fornices. Laterally, the uterine arteries course medially within the base of the broad ligament to reach the cervix and uterine body, where they anastomose with branches of the vaginal artery along the upper vagina. Muscular architecture of the vaginal wall is emphasized, with an inner circular and outer longitudinal smooth muscle arrangement surrounding the lumen and continuous superiorly with cervical stroma. Attention to the shared vascular supply matters when you are planning cervical procedures and vaginal surgery, because the descending cervical and vaginal branches sit close to the lateral fornix and can be a source of brisk hemorrhage if not anticipated.
