Description
Arising from the left lower lobar (inferior) bronchus, the superior segmental bronchus (B6) courses posterolaterally and superiorly toward the apical portion of the lower lobe. From a left lateral aspect, its lumen and cartilaginous wall sit deep to the visceral pleura, with the bronchus lying posterior to the expected course of the inferior pulmonary vein and inferior to the upper lobe bronchial divisions. Segmental branches would be directed into the superior segment (S6), oriented toward the upper, apical contour of the lower lobe rather than the true lung apex beneath the cupula. Orientation is the point. B6 is a frequent landmark in bronchoscopy and thoracic surgery because it defines the superior segment of the lower lobe, a target in segmentectomy for localized malignancy and in resection of post-obstructive infection. Its upward trajectory also explains why aspirated material and mucus plugging can track into dependent posterior basilar segments when supine, while secretions in upright patients may pool differently across lower-lobe segments. Left-sided anatomy matters, since the left main bronchus is longer and more oblique, and adjacent hilar relationships constrain stapling planes and lymph node dissection.
