Description

Seen from the left side of the male left lung, the superior lingular segment (segmentum lingulare superius, S4) is color-coded within the superior lobe, forming the anteroinferior tongue-like projection known as the lingula. Its segmental territory lies inferior to the anterior segment (S3) and anterior to the inferior lingular segment (S5), abutting the cardiac impression medially along the anterior border. The oblique fissure courses posteroinferiorly, separating the superior lobe from the inferior lobe and framing the lingular segments along the fissural surface. Isolating S4 matters because the lingula functions as the left-sided counterpart of the right middle lobe, and segment-level anatomy often guides how you describe disease distribution on CT and how you plan resection. Focal infection or atelectasis in the lingula can mimic lower lobe pathology on frontal radiographs, while bronchoscopy and segmentectomy planning rely on recognizing the lingular bronchus branching from the left upper lobe bronchus and the accompanying segmental pulmonary arterial supply. This is a common site of postoperative confusion. Clear segment boundaries reduce it.
