Description
Colored rendering isolates the apicoposterior segment (segmentum apicoposterius, S1+S2) of the left upper lobe, positioned at the superior pole of the lung and wrapping posteriorly toward the vertebral column. Its apex rises above the level of the first rib into the thoracic inlet, while the segment’s posterior aspect lies closer to the posterior chest wall than the anterior segment (S3). Medially, this segment approaches the left hilum where the segmental bronchus and pulmonary arterial branches course toward it, with the oblique fissure forming the inferoposterior boundary with the lower lobe. Segmental anatomy matters most when you need to localize disease to a bronchopulmonary unit. Apicoposterior involvement is common in reactivation tuberculosis and in aspiration patterns affected by patient position, and it is a frequent target in oncologic workups where a small peripheral nodule must be assigned to a resectional segment rather than the entire lobe. For thoracic surgeons and interventional pulmonologists, separating S1+S2 from adjacent territories clarifies the logic of a left upper-lobe segmentectomy, including where intersegmental planes are expected and why air leaks tend to occur along fissural and subpleural boundaries.
