Description
Posterior thoracic anatomy frames the lower lobes, with Segmentum basale laterale (S9), the lateral basal segment, highlighted in color on each lung and occupying the inferolateral portion of the diaphragmatic surface. From this back view, S9 sits lateral to the posterior basal segment (S10) and inferior to the superior segment of the lower lobe (S6), wrapping toward the costal pleura along the posterolateral rib cage. The left-sided emphasis places the colored segment against the left lower lobe, where the oblique fissure forms a superior boundary as the segment tapers toward the hilum. S9 matters because it is one of the most dependent bronchopulmonary segments in upright patients and a common reservoir for aspirated material, secretion retention, and basilar atelectasis, patterns that often localize to the posterior and lateral basal segments on chest radiography and CT. Segmental anatomy also guides bronchoscopic navigation and informs a limited resection, such as a lower-lobe basilar segmentectomy, where intersegmental planes follow the segmental bronchi and pulmonary arteries rather than lobar fissures. A frequent pitfall in teaching is confusing basilar segment distribution between right and left lungs. This posterior view reduces that ambiguity.
