Description
Oriented from an inferior perspective, the lateral basal bronchopulmonary segments (segmentum basale laterale, S9) occupy the posterolateral portion of each lower lobe, cupped against the diaphragmatic surface and curving into the sharp inferior border of the lung. The segment lies lateral to the medial basal segment (S7) and anterior to the posterior basal segment (S10), with its pleural surface facing the costodiaphragmatic recess laterally. Segmental boundaries follow the expected intersegmental planes defined by pulmonary veins, while the segmental bronchus and artery course centrally within the parenchyma. Teaching lower lobe segmentation from below matters because the diaphragmatic surface is where dependent disease accumulates and where the clinician’s mental map often fails during procedures. Aspiration pneumonia and dependent atelectasis frequently involve the basal segments in recumbent patients, and an accurate grasp of S9 helps interpret basilar opacities on chest radiographs and CT when the question is whether changes are lateral versus posterior.
