Oriented to the right hemithorax, this plate isolates the lateral segment (Segmentum laterale, S4) of the right middle lobe and its relationship to the surrounding pulmonary architecture. The segment occupies an anterolateral position, lying inferior to the horizontal fissure and anterior to the oblique fissure, with its peripheral parenchyma approaching the costal pleura. Medially, the segmental bronchus and accompanying pulmonary artery branch course toward the hilum, while the segment’s venous drainage runs more intersegmentally toward the right superior pulmonary vein. Clean boundaries matter. Segmental anatomy in the right middle lobe guides bronchoscopy and resection planning because S4 and the medial segment (S5) can behave differently in infection and obstruction. Right middle lobe syndrome often reflects impaired drainage or extrinsic compression, and this lateral segment view helps clarify why atelectasis may persist when the middle lobe bronchus is narrowed.
