Description
Positioned in a right lateral view, the right lung is segmented to emphasize the lateral basal bronchopulmonary segment (Segmentum basale laterale, S9) within the inferior lobe. The highlighted S9 territory lies inferior to the horizontal fissure and posterior to the middle lobe’s lateral surface, forming a substantial part of the posterolateral diaphragmatic (basilar) lung. Its parenchyma abuts the costal pleura laterally and slopes inferiorly onto the diaphragmatic pleura, with its segmental bronchus and artery coursing centrally toward the hilum while intersegmental veins track along the boundaries. Segmental anatomy matters most when you need to localize disease beyond the lobe level. The lateral basal segment is a frequent site of dependent atelectasis and aspiration-related opacities in supine patients because it sits low and posterolateral, and its bronchial orientation can favor secretion pooling. Thoracic surgeons also think in terms of S9 during video-assisted thoracoscopic (VATS) segmentectomy, where preserving intersegmental veins and accurately defining the inflation-deflation line helps achieve an oncologic margin for small peripheral lesions while sparing lung function. Pulmonology and radiology educators can use this right side perspective to teach bronchopulmonary segment nomenclature and to correlate a lateral chest radiograph or axial CT findings with a concrete surface territory on the inferior lobe. It also fits well in operative atlases, preoperative planning materials, and patient-facing explanations of targeted resection in early-stage non-small cell lung cancer.
