Description
Positioned at the apex of the right superior lobe, the apical segment (segmentum apicale, S1) occupies the superomedial portion of the lung and tapers toward the thoracic inlet. From a posterior-lateral viewpoint, its pleural surface is continuous with the visceral pleura, curving over the domed apex and descending inferiorly toward the posterior part of the oblique fissure. Medially, the segment approaches the mediastinal pleural reflection, while laterally it rounds toward the costal surface under the upper ribs. Boundaries implied in this perspective include the posterior segment (S2) inferior and posterior to S1, with intersegmental planes running between their bronchovascular territories. Segmental anatomy matters most when you need to talk about disease in a way that maps to bronchi and vessels, not just lobes. A solitary pulmonary nodule in the apical segment on CT is handled differently than a diffuse upper-lobe process, and the posterior aspect is a common route of spread for apical infections and post-primary tuberculosis in the right upper lobe. Surgeons planning a right upper-lobe segmentectomy rely on accurate S1 topography to anticipate intersegmental veins, define resection planes, and avoid leaving an ischemic remnant. Small segment.
