Description
Seen from a superior orientation, the right lung is presented with the apical segment (segmentum apicale, S1) color-coded at the apex of the superior lobe. Its pleural surface slopes inferiorly from the cupola toward the horizontal fissure, with the mediastinal surface lying medial and the costal surface lateral. Immediately inferior and posterior to S1, the posterior segment (S2) and anterior segment (S3) would be expected within the superior lobe, separated internally by segmental bronchi and pulmonary arterial branches even when surface landmarks are subtle. Surgeons and bronchoscopists care about this angle because the apical segment sits closest to the thoracic inlet, where the right upper lobe bronchus branches early from the right main bronchus and divides into segmental bronchi (B1, B2, B3) that guide segmentectomy planes. Apical and apicoposterior disease distributions matter: post-primary tuberculosis and smoking-related upper lobe emphysema commonly favor the lung apices, and small peripheral adenocarcinomas in S1 often prompt discussion of anatomic segmentectomy versus lobectomy with attention to the segmental veins along the intersegmental plane.
