Description
Viewed from the right side, the lungs are rendered with emphasis on the superior (apical) segments of the inferior lobes, highlighted in color against the remaining basal segments. These segmental territories occupy the posterior and superior portion of each lower lobe, lying dorsal to the oblique fissure and tapering toward the posterior thoracic wall. The right lower lobe is presented lateral to the mediastinal contour, while the segmental apex sits superior to the diaphragmatic surface and posterior to the expected course of the hilum and pulmonary ligament. Segmental anatomy matters most when you need to talk about disease distribution and targeted therapy. The superior segment of the lower lobe is a classic site for aspiration-related consolidation in a supine patient and for posteriorly dependent atelectasis, patterns that differ from upright aspiration into the basal segments. Surgeons and interventional pulmonologists also reference this segment during segmentectomy planning and bronchoscopy, where the segmental bronchus (B6) and its accompanying segmental artery define resection planes and help distinguish apical lower-lobe disease from upper-lobe pathology on radiographs and CT.
