Presented in medial view, the talus is oriented in anatomical position to isolate the medial malleolar facet on the medial surface of the trochlea tali. The comma-shaped articular surface tracks along the superomedial aspect of the talar body, lying superior to the sustentacular region and anterior to the posterior process. As the camera subtly rotates and settles, adjacent landmarks come into relief, including the talar neck anteriorly and the transition into the talar dome laterally. This facet forms the talar side of the medial tibiotalar articulation with the medial malleolus of the tibia, a contact area that becomes clinically relevant in ankle fracture patterns and post-traumatic arthritis. Medial malleolar fractures, talar dome osteochondral lesions, and malreduction after syndesmotic injury can all alter load transfer across the medial gutter, and small incongruities are easy to miss in static teaching plates.
