Description
Viewed from the medial aspect, the stomach (gaster, ventriculus) is covered by a continuous gastric serosa (visceral peritoneum) that reflects from the organ onto adjacent peritoneal folds. The serosal surface follows the contour of the lesser curvature medially and sweeps inferiorly toward the greater curvature, maintaining a smooth, glistening interface between the gastric wall and the abdominal cavity. Proximally, the coating continues over the cardia near the esophagogastric junction, then courses distally across the body toward the pyloric antrum and pylorus. Orientation is anchored by the superior fundus beneath the left hemidiaphragm and the more inferior, anteriorly sloping antrum. Medial presentation of the serosa matters because it clarifies where the stomach is intraperitoneal and how peritoneal reflections define surgical and pathological planes. During mobilization along the lesser curvature, surgeons work in the serosal plane when opening the lesser omentum (gastrohepatic ligament) to access the lesser sac and control vessels near the pylorus. Peritoneal irritation at this surface also explains why perforated peptic ulcer disease or gastric carcinoma with serosal invasion can rapidly seed the peritoneum, producing localized guarding or diffuse peritonitis depending on contamination and spread.
