Description
Centered on a sagittal midline section, the uterine cavity appears as a narrow, triangular lumen within the thick myometrium, with the endometrial lining forming the immediate boundary of the space. Superiorly, the cavity tapers toward the uterine fundus and the tubal ostia region, while inferiorly it narrows into the uterine isthmus and continues as the cervical canal. Anterior and posterior uterine walls flank the lumen, with the anterior wall oriented toward the bladder and the posterior wall toward the rectouterine pouch. Sagittal anatomy of the uterine cavity matters because so much gynecologic pathology and instrumentation follows this axis. The view clarifies the relationship between the endometrial cavity, isthmus, and cervix, which is the corridor traversed during endometrial biopsy, dilation and curettage, and hysteroscopy. It also supports teaching of common distortions of the cavity, such as leiomyomas that indent the endometrium, intrauterine adhesions (Asherman syndrome), and congenital Müllerian anomalies like a septate uterus, all of which change the expected triangular contour on hysterosalpingography or sonohysterography.
