Description
Seen in transverse section through the right ventricular outflow tract, the pulmonary trunk lumen is circumscribed by the sinovalvar (valvular sinus) region inferiorly and the sinotubular junction superiorly, where the supravalvular ridge (supravalvular crest) forms a subtle circumferential thickening. The pulmonic valve cusps sit inferior to this ridge, their attachments defining the transition from the right ventricular infundibulum to the arterial wall. Anteriorly and leftward, the pulmonary trunk occupies its expected position relative to the ascending aorta, while the ridge tracks concentrically along the arterial circumference at the level just above the commissures. A small structure, but a real landmark. That short segment of pulmonary root is where morphology matters when you are separating valvar stenosis from supravalvular narrowing, whether on echocardiography, CT angiography, or in the operating room during pulmonary root reconstruction. The sinotubular junction sets the geometry that governs cusp coaptation; even modest ridge prominence can alter effective orifice area and create high-velocity jets that mimic primary pulmonic valve disease. For congenital discussions, this is the reference level for describing supravalvular pulmonary stenosis and postoperative caliber mismatch after repair of tetralogy of Fallot or arterial switch-related neopulmonary root changes.
