Description
Prominent transverse rectal folds (valves of Houston) project into the lumen of the male rectum as semilunar mucosal ridges. Typically three in number, they appear staggered in a spiral pattern, with superior and inferior folds alternating from the left and right lateral walls and a middle fold often arising from the opposite side, narrowing the central passage in an anteroposteriorly flattened tube. Superiorly, the rectum continues into the sigmoid colon, while inferiorly it transitions toward the anorectal junction where the ampulla gives way to the anal canal. Peritoneal reflection and the relationship to adjacent pelvic organs sit anterior to the rectum, but the folds themselves are defined by mucosa and submucosa draped over the underlying muscularis. Recognition of these transverse rectal folds matters in endoscopy and rectal instrumentation because they are fixed landmarks, not transient haustral contractions. The middle Houston valve can resist advancement of a rigid sigmoidoscope and is a common point where the instrument tip catches if the lumen is not followed along the natural curves.
