The round ligament tracks superiorly within the umbilical fissure (fissura ligamenti teretis), marking the division between the left medial and left lateral segments on the visceral surface. Superior to this region, the convex diaphragmatic surface of the right and left lobes forms the dominant contour, while the falciform ligament attachment aligns toward the anterior surface and the umbilical portion of the left portal vein deep to the fissure. Recognition of the notch for ligamentum teres matters because it is a dependable surface landmark for orienting the liver during open or laparoscopic exploration when the visceral surface is only partially visualized. The ligamentum teres represents the fibrotic remnant of the fetal left umbilical vein and can recanalize in portal hypertension, becoming a prominent paraumbilical collateral that courses toward the anterior abdominal wall (caput medusae).
