Description
Arising from the ventral midbrain, the oculomotor nerve (CN III) is shown emerging in the interpeduncular fossa between the cerebral peduncles, then coursing anteriorly toward the parasellar region. Its rootlets sit medial to the cerebral peduncle and pass in close relationship to the posterior cerebral artery superiorly and the superior cerebellar artery inferiorly, a corridor clinicians often reference when localizing compressive lesions. From an anterior perspective, CN III can be followed toward the cavernous sinus and onward to the superior orbital fissure, where its superior and inferior divisions distribute to extraocular muscles and carry preganglionic parasympathetic fibers to the ciliary ganglion. Small but unforgiving anatomy. This view matters because CN III palsy is one of the quickest cranial nerve patterns to triage, and the anatomy in the interpeduncular cistern explains why. A posterior communicating artery aneurysm commonly compresses the superficial parasympathetic fibers, producing early mydriasis and impaired accommodation, while microvascular ischemia in diabetes or hypertension more often spares the pupil and selectively weakens the levator palpebrae superioris and extraocular muscles. The midbrain exit zone also frames teaching on Weber syndrome and adjacent peduncular lesions, where ipsilateral oculomotor deficits accompany contralateral hemiparesis. Neuroanatomy and neurology educators can pair this plate with cavernous sinus and orbital diagrams to teach lesion localization from brainstem to orbit, and it fits cleanly into aneurysm, diplopia, and pupillary abnormality chapters in clinical texts.
