Description

Arising from the anterior division of the internal iliac artery, the uterine artery courses medially along the pelvic sidewall within the base of the broad ligament toward the cervix and uterine isthmus, then ascends tortuously along the lateral uterine margin toward the fundus. Near the level of the cervix it crosses superior to the ureter, a short segmental relationship that anchors the classic water under the bridge landmark. Anastomoses with the ovarian artery appear superiorly near the uterine tube and cornual region, while descending cervicovaginal branches project inferiorly toward the upper vagina. Tortuosity and caliber change across the menstrual cycle, and the external configuration becomes even more pronounced in pregnancy as uterine blood flow rises and collateral pathways expand. That morphology matters in hemostatic surgery: uterine artery ligation at the level of the cervicoisthmic junction, stepwise devascularization, and internal iliac artery ligation all depend on identifying the uterine artery before it disappears into the parametrium. Uterine artery embolization for symptomatic fibroids also hinges on recognizing expected variants and anastomoses, since reflux into ovarian branches can compromise ovarian reserve.
