Description
Arcuate arteries course circumferentially within the myometrium, running in the uterine wall from lateral toward the medial fundus and body, with radial arteries piercing inward toward the endometrium. Tortuous uterine arteries approach the uterus at the level of the cervix from the pelvic sidewall, then ascend along the lateral uterine margin within the broad ligament before giving off multiple coiled branches. Spiral arteries extend distally into the endometrium, where their length and coiling increase toward the functional layer. Coiling is the point. Understanding this arrangement matters in uterine-sparing and obstetric settings because the uterine artery’s ascending course and branching pattern determine where hemorrhage control succeeds or fails. During hysterectomy and uterine artery ligation, the artery is classically secured near the cervix, just superior to the ureter as it passes beneath the uterine artery (water under the bridge), and this relationship drives ureteric injury risk when anatomy is distorted by fibroids, endometriosis, or prior surgery.
