Description
Oriented in the normal anteflexed position, the uterus sits in the midline of the lesser pelvis with the fundus directed superiorly and anteriorly over the urinary bladder and the cervix angled inferiorly toward the vagina. The uterine body (corpus uteri) tapers into the isthmus, and the fundal region expands laterally into the right and left uterine horns (cornua uteri), where the uterine tubes would originate in continuity with the endometrial cavity. Anterior and posterior surfaces are implied by the anteflexion, with the anterior uterine wall facing the vesicouterine space and the posterior wall relating to the rectouterine pouch (pouch of Douglas). Horn position is a key landmark. This orientation matters because anteversion and anteflexion determine how the uterus lies relative to the bladder and rectum during bimanual examination, transvaginal ultrasound, and instrumentation. A retroverted or retroflexed uterus can change the uterine axis, complicate uterine sounding, and increase the risk of perforation during dilation and curettage or IUD placement. Fundal contour and the separation of the uterine horns also frame common congenital variants (bicornuate or septate uterus) that affect fertility and obstetric outcomes.
