Posterior surface anatomy of the pancreas is rendered with emphasis on the omental eminence (tuber omentale pancreatis), a localized convex projection along the superior border that abuts the posterior wall of the lesser sac. Superior to the gland, the posterior surface relates to the omental bursa and the peritoneal reflections of the lesser omentum, while inferiorly the pancreatic body approaches the root of the transverse mesocolon. Medially the gland broadens toward the head near the duodenal C loop, and laterally it tapers toward the tail as it courses toward the splenic hilum. Topography matters. Attention to the omental tubercle is useful when teaching why peripancreatic fluid collections and posteriorly extending pancreatitis can dissect into the lesser sac and pararenal spaces, producing patterns that are predictable once the posterior relations are understood.
