Description
Posteriorly oriented anatomy centers on the male transverse colon as it spans the mid-abdomen from the right colic (hepatic) flexure to the left colic (splenic) flexure. Inferior and to the right, the cecum continues into the ascending colon along the right flank, then turns sharply superiorly beneath the liver before transitioning into the transverse segment. More to the left, the colon approaches the splenic region, where the flexure sits higher and more posterior relative to the left upper quadrant viscera. A continuous large-intestine outline is implied rather than an isolated segment. For teaching and operative planning, a posterior observation point clarifies why the transverse colon is a mobile segment, suspended by the transverse mesocolon, while the ascending colon is secondarily retroperitoneal and comparatively fixed. That contrast matters during colonoscopy and laparoscopic colectomy, when looping tends to occur across the transverse colon and when mobilization planes differ at the hepatic and splenic flexures. The splenic flexure also marks a watershed between superior and inferior mesenteric arterial territories, a common concept when discussing ischemic colitis at Griffiths’ point and segmental hypoperfusion in low-flow states. Orientation matters.
