Description
Posterior to the pancreatic head and proximal body, the narrow neck of the pancreas (collum pancreatis, isthmus) spans transversely across the midline, with its dorsal surface closely applied to the superior mesenteric vein as it joins the splenic vein to form the portal vein. Inferior to the neck, the superior mesenteric artery emerges from behind the gland and descends into the root of the mesentery, while the uncinate process lies posteromedial and hooks behind the mesenteric vessels. A short segment of the main pancreatic duct courses within the parenchyma toward the head. Tight real estate. Surgeons focus on the pancreatic neck because it is the usual plane for transection in pancreaticoduodenectomy, and the portal venous confluence sits directly posterior, leaving little margin when mobilizing the gland from the prepancreatic fascia. Venous injury here can rapidly become a major hemorrhage, and preoperative planning often hinges on whether a pancreatic head tumor abuts or encases the SMV-portal vein confluence, determining the need for venous resection and reconstruction. This dorsal relationship also explains why posterior penetrating ulcers or pancreatitis-related inflammation can involve portal or superior mesenteric venous thrombosis. Use this posterior perspective in upper abdominal anatomy teaching to clarify the three-dimensional relationship of pancreas neck, SMV, splenic vein, and portal vein, and in hepatopancreatobiliary surgery atlases or operative consent materials describing pancreatic neck transection and venous control. It also suits radiology education when correlating axial CT or portal venous phase imaging with surgical anatomy around the prepancreatic region.
