Pulmonary Hilum Containing Main Bronchi and Vessels

Description

Arising from the mediastinal surface of the left lung, the pulmonary hilum (radix pulmonis) clusters the left main bronchus, the left pulmonary artery, and the superior and inferior pulmonary veins as they enter and leave the lung at the hilar depression. Superior within the root, the pulmonary artery typically lies above the bronchus on the left, while the pulmonary veins occupy a more anterior and inferior plane relative to the bronchial lumen. Branching distally, the left upper lobe bronchus gives off the apicoposterior segmental bronchus within the superior division, coursing superiorly and posteriorly toward the apical and posterior segments. Anatomists and clinicians return to the hilum because small shifts in this crowded arrangement change both interpretation and technique. Left sided relationships matter in thoracic imaging and endobronchial navigation, where the apicoposterior segment is a common target for bronchoscopy, bronchoalveolar lavage, or segmental resection planning. Lymph nodes along the bronchus and pulmonary vessels (hilar and bronchopulmonary nodes) explain classic patterns of spread in lung carcinoma and sarcoidosis, and they sit where surgeons and interventionalists must work.
