Description
Medial inspection of the right lung centers on the hilum (hilus pulmonis) and adjacent mediastinal pleura, where the root of the lung crosses the organ boundary. The right main bronchus lies posterior within the hilum, while the pulmonary artery is positioned anterior to it and the superior and inferior pulmonary veins occupy the most anterior and inferior planes. Superiorly, the apex of the hilum blends into the pleural sleeve of the pulmonary ligament, and inferiorly the ligament descends toward the diaphragmatic surface, marking the caudal limit of the root. Color-coding of the hilum is useful because the right-sided relationships are asymmetric and clinically unforgiving: the right pulmonary artery sits anterior to the bronchus, and the eparterial (superior lobar) bronchus typically arises above the artery, a pattern remembered in thoracic surgery when distinguishing bronchial branches at the root. Small shifts matter. On CT and during bronchoscopy, conflating artery, vein, and bronchus at the hilum can lead to misidentification of a right hilar mass, lymphadenopathy, or vascular enlargement, and the mediastinal view clarifies why right hilar nodes can sit tightly around the bronchus and pulmonary artery.
