Anterior thorax and proximal upper limbs are presented on an adult male figure, with a nonblanching purpuric eruption scattered across the upper chest and extending laterally toward the shoulders. Lesions range from pinpoint petechiae to larger purpuric macules and patches, coalescing over the pectoral region and remaining superficial to the dermis and subcutis. The distribution is predominantly anterior and superior on the trunk, sparing the lower abdomen by comparison, and reads as hematogenous skin hemorrhage rather than an erythematous, pressure-blanching exanthem. Color contrast is clear. Meningococcal sepsis from Neisseria meningitidis classically produces a rapidly evolving petechial to purpuric rash due to endotoxemia, vascular injury, and disseminated intravascular coagulation, and the key bedside discriminator is exactly what is visualized here: lesions that do not fade with pressure.
