Description
Scattered rose spots are visible on the anterior trunk of an adult male, presenting as discrete, slightly raised salmon pink macules and maculopapules across the upper chest and epigastrium. Lesions sit superficially within the skin and are separated by intervening normal integument, a distribution that reads as truncal and relatively symmetric rather than dermatomal. Color and size variation suggests different lesion ages, with faint pink early macules and darker erythematous spots more laterally. No vesiculation is implied. Rose spots are a classic cutaneous sign of enteric (typhoid) fever from Salmonella enterica serovar Typhi during bacteremia, often appearing in the second week as brief crops on the trunk and proximal abdomen, then fading within days. Their clinical value is timing and context: a febrile patient with abdominal pain, constipation or diarrhea, relative bradycardia, and possible hepatosplenomegaly makes this rash more than a generic viral exanthem. Recognizing the morphology helps narrow the differential from drug eruptions and pityriasis rosea to meningococcemia (typically more purpuric and toxic), and it supports prompt blood cultures and targeted antimicrobial therapy while considering complications such as intestinal hemorrhage or ileal perforation.
