Description
Arising just inferior to the carina, the right main bronchus courses inferolaterally and gives off the right superior lobar bronchus before continuing as the bronchus intermedius toward the right inferior lobar bronchus. From an anterior vantage point, the right inferior lobar bronchus is seen descending posterior to the pulmonary artery and approaching the hilum, where it will divide into segmental bronchi for the lower lobe. Medially, the airway remains continuous with the trachea, while laterally it projects toward the bulk of the right lung. Orientation at the tracheal bifurcation matters in bronchoscopy and in interpreting CT of the central airways. The right main bronchus is shorter, wider, and more vertical than the left, so aspirated material tends to pass into the right bronchial tree and can track into the lower lobe bronchi when patients are supine. That geometry also frames lymph node mapping, with the carina and proximal lobar bronchi bordering stations commonly sampled by endobronchial ultrasound for lung cancer staging. Use this illustration in gross anatomy and respiratory modules to teach branching patterns (right main bronchus to bronchus intermedius to right inferior lobar bronchus) and to anchor discussions of aspiration pneumonia distribution and central airway obstruction.
