Description
Framed from an anterior profile of the thorax, the trachea descends in the midline and bifurcates at the carina into the right and left main (primary) bronchi. Each main bronchus then divides into lobar (secondary) bronchi, with the right superior, middle, and inferior lobar bronchi arising in a more vertical, shorter bronchial tree than the longer, more oblique left main bronchus. Distal branching toward the segmental (tertiary) bronchi is implied as the airway narrows and courses inferolaterally within each lung hilum. Orientation is anterior, so cartilaginous airway contours and the branching geometry take priority over posterior mediastinal relationships. An anterior profile clarifies why aspirated material most often tracks into the right bronchial system, given the right main bronchus alignment relative to the trachea and the early takeoff of the right upper lobar bronchus. This is the branching pattern you reference when correlating lobar collapse on chest radiography, planning bronchoscopy to sample a specific lobe, or interpreting mucus plugging that obstructs a lobar bronchus. Small shifts in angle matter. They change where a scope tip goes and where secretions pool.
