Description

Right lung bronchi are presented in a rightward lateral orientation, centering on the lateral segmental bronchus (B4) as it courses into the middle lobe. The bronchus arises distally from the right middle lobar bronchus, branching anterolaterally away from the bronchus intermedius while remaining anterior and inferior to the right upper lobar bronchus. Its segmental distribution tracks toward the lateral pleural surface of the middle lobe, with neighboring middle lobe anatomy implied by the adjacent medial segmental bronchus (B5) and the interlobar fissural planes. Clear spatial hierarchy. Proximal airway to distal segment. Middle lobe segmental anatomy matters because B4 frequently defines the drainage and disease boundaries that clinicians rely on when middle lobe pathology refuses to clear. Recurrent right middle lobe atelectasis or infection can reflect obstruction at the lobar or segmental level, including mucus plugging, endobronchial tumor, or extrinsic compression by peribronchial and hilar lymph nodes, and the lateral segment may show characteristic collapse patterns on radiography and CT. During bronchoscopy, identifying the takeoff of the right middle lobar bronchus and then selecting B4 guides targeted lavage, biopsy, or culture sampling without contaminating adjacent segments. Pulmonology and thoracic surgery educators can place this plate into airway anatomy teaching, segmental bronchopulmonary localization, and preoperative planning discussions for middle lobe segmentectomy versus lobectomy. It also suits figure panels for bronchoscopy manuals, radiology texts correlating bronchi to bronchopulmonary segments, and patient-facing materials explaining why a lesion can be confined to the lateral middle lobe. Anatomical accuracy verified by SciePro's Medical Advisory Board.
