Description
Arcing from the interventricular septum into the right ventricular cavity, the anterior limb of the septomarginal trabecula (moderator band, trabecula septomarginalis) is presented in sectional cut, with its muscular bundle seen spanning toward the anterior papillary muscle. Surrounding it, the trabeculated right ventricular free wall lies lateral, while the smoother septal surface sits medial and posterior to the band’s origin. The band’s course is framed within the spacious right ventricle, where its relationship to adjacent septal trabeculations and the ventricular lumen can be appreciated in cross-section. Clinically, this is a conduction landmark, not just a trabecula: the moderator band commonly carries a component of the right bundle branch to the anterior papillary muscle, anchoring discussions of right ventricular activation and arrhythmogenic substrates. Surgeons and imagers care about it because a prominent band can mimic an intracavitary mass or thrombus on echocardiography, and in repaired congenital heart disease (for example tetralogy of Fallot) right ventricular muscle bundles and the moderator band can contribute to or be mistaken for obstructing tissue in the right ventricular outflow tract. Orientation matters. A sectional view helps distinguish a true muscular bridge from adjacent trabeculae carneae by showing the band’s discrete profile and attachment points.
