Standard Representation of the Male Ascending Colon

Description

Running superiorly along the right lateral abdominal wall, the ascending colon (colon ascendens) extends from the caecum in the right iliac fossa toward the hepatic flexure beneath the inferior margin of the liver. Haustra coli segment the wall into sacculations, with taeniae coli tracking longitudinally and converging proximally toward the caecum. Peritoneal reflections along the lateral border form a paracolic gutter, while the posterior surface lies in close apposition to retroperitoneal structures, classically including the right kidney and ureter. This is the right-sided, proximal large intestine. Clinically, this segment is a common site for right-sided colon cancer, where a larger luminal caliber and liquid stool can delay obstructive symptoms and shift presentation toward iron-deficiency anaemia. Its peritoneal relationships also matter during a right hemicolectomy, when mobilization along the white line of Toldt releases the ascending colon from the lateral peritoneal attachments and exposes the retroperitoneum, including the duodenum and ureter, for protection. Orientation at the hepatic flexure guides both surgical planes and endoscopic navigation, and helps explain why inflammatory or neoplastic processes can track along the right paracolic gutter.
