Description

Running along the radial side of the male forearm, the abductor pollicis longus is highlighted beneath the skin in lateral profile, with the radius and ulna forming the deep skeletal scaffold from the elbow to the wrist. Proximally, the muscle belly lies posterior and lateral, arising from the posterior surfaces of the ulna and radius and the interosseous membrane, then passing distally toward the radial styloid. Its tendon courses anterior to the lateral edge of the distal radius, entering the first extensor compartment deep to the extensor retinaculum and heading toward the base of the first metacarpal, lateral to the carpal bones and proximal to the thumb metacarpophalangeal joint. Bony landmarks are clear. Metacarpals and phalanges anchor the distal orientation. This lateral arrangement matters because the abductor pollicis longus tendon, commonly sharing the first dorsal compartment with extensor pollicis brevis, is the structure most often implicated in De Quervain tenosynovitis, where thickening at the radial styloid produces pain with thumb abduction and ulnar deviation. Surgeons and sonographers use this relationship to distinguish compartmental disease from intersection syndrome, which occurs more proximally where the first compartment tendons cross the extensor carpi radialis longus and brevis. The tendon also defines part of the anatomical snuffbox boundary, a practical surface landmark when considering radial artery access and scaphoid tenderness. Hand and upper-limb anatomy courses can use this plate to teach the transition from posterior forearm musculature to the thumb rays, and to reinforce how tendons redirect at the extensor retinaculum.
