Description

Viewed laterally on a male head and neck model, the spinal accessory nerve (cranial nerve XI) emerges deep to the sternocleidomastoid, then courses obliquely posteroinferiorly across the posterior cervical triangle toward the trapezius. Superficial to the prevertebral fascia and deep to the investing layer of deep cervical fascia, it travels between the posterior border of the sternocleidomastoid anteriorly and the anterior border of the trapezius posteriorly. Adjacent venous and arterial structures provide orientation in the carotid and posterior triangle regions, while the mandible, mastoid region of the temporal bone, and upper cervical vertebrae anchor the pathway in skeletal context. Clear landmarks. This lateral perspective matters because it matches the operative corridor where the accessory nerve is most exposed: the posterior triangle. Iatrogenic injury during cervical lymph node biopsy, neck dissection (levels II to V), or trauma to the posterior border of the sternocleidomastoid classically produces trapezius weakness with shoulder droop, impaired abduction above 90 degrees from loss of scapular upward rotation, and lateral scapular winging that is distinct from long thoracic nerve palsy. Identifying where the nerve exits the sternocleidomastoid and crosses the posterior triangle also helps differentiate accessory neuropathy from C3 to C4 contributions to the cervical plexus that can partly mask deficits.
