Description
Originating at the medulla oblongata, the hypoglossal nerve (cranial nerve XII) exits the skull base through the hypoglossal canal and descends in the lateral neck before turning anteriorly toward the tongue. From a lateral perspective you would expect it to pass inferior to the posterior belly of the digastric and stylohyoid, then course forward superficial to the hyoglossus and deep to the mylohyoid as it enters the sublingual region. Along that path it runs in close company with the internal carotid artery and internal jugular vein proximally, then crosses the external carotid system near the origins of the occipital and lingual arteries. A small but practical landmark. This angle matters because CN XII is a workhorse motor nerve and its extracranial course explains both its exam findings and its iatrogenic risk points. Injury at or near the hypoglossal canal, or along the carotid bifurcation during carotid endarterectomy, can produce ipsilateral tongue weakness with deviation toward the lesion and dysarthria, while more distal injury during submandibular gland surgery risks isolated tongue motor deficits. The lateral relationship to the hyoid region also helps clarify why deep neck infections and oropharyngeal tumors can involve hypoglossal function without primary tongue pathology. Faculty can drop this into head and neck anatomy blocks to teach cranial nerve exit foramina and the suprahyoid floor of mouth, and surgeons will find it fits well in figures for carotid surgery, submandibular approaches, and skull base discussions. It also pairs cleanly with neuroanatomy content on lower motor neuron tongue findings and with radiology teaching on lesion localization around the hypoglossal canal.
