Description
Centered in the female pelvis, the uterus is rendered with ectopic endometrial implants and fibrotic endometriotic lesions along the serosal surface, with deeper nodules tracking into adjacent supporting tissues. Posteriorly, thickening and tethering extend toward the rectouterine pouch (pouch of Douglas) and uterosacral ligaments, where deeply infiltrating endometriosis commonly concentrates. Superior and lateral relationships to the adnexa are implied, since ovarian surface implants and peri-tubal adhesions often accompany the uterine disease pattern and distort normal tubo-ovarian mobility. Distorted contours and scarring dominate. Deep infiltrating endometriosis matters because it changes pelvic anatomy in ways that directly drive symptoms and operative risk, not just histologic diagnosis. Nodules in the uterosacral ligament complex correlate with deep dyspareunia and chronic pelvic pain, while posterior compartment disease can fix the uterus in a retroverted position and narrow the rectovaginal space, complicating laparoscopic dissection. When adhesions bridge the posterior uterus to the sigmoid colon or rectum, surgeons must anticipate ureteral proximity at the pelvic brim and near the uterine artery, and infertility workups often hinge on understanding how peri-tubal fibrosis and ovarian adhesions block ovum pickup despite a patent lumen.
